    Then Go to the Zonal Public Distribution office and collect Form 3 for Changing the name on the ration card

  4. Now file Form 3 and attach the affidavit and Newspaper cutting

    Also, the proof of Ration card should be submitted

  5. Submit the form

    Submit the form and an Acknowledgement slip will be given to check the status

By following their process and submitting the form it takes more than a month and when the authority verifies the details then-new ration card with a name change will be issued.
