Firstly the Procedure to be followed to change the Name in the ration card is a bit lengthy process, If it is mandatory to change the name then only you can take the step forward. SO follow the below steps o change the Name in the Ration card.

  1. Firstly get the Ration card Name change Application form

    Make an Affidavit from a lawyer for Name Change

  2. You need to give paper post for Name Change in two local and national newspaper

    An appeal in Court that Old and New Name belongs to the same person with proof as Paper cutting and Affidavit

  3. Now Fill the Application form correctly with new Name

    Mention your same old address and other details in the form

  4. Attach the Affidavit, Newspaper cutting with Court order to the Application form

    Now submit the Application form with attached documents to the Directorate of Food, Civil Supplies, and Consumer affair department

  5. Take the enrollment slip

    Take the enrollment slip which will direct you to receive the Ration card from a particular officer
